Demystifying Endoscopic Spine Surgery: A Modern Approach to Back Pain

We are now able to perform complex spinal procedures using highly advanced, minimally invasive techniques known as endoscopic spine surgery. By utilizing tiny incisions and high-definition cameras, we can effectively treat the root cause of your back or leg pain without the severe tissue disruption associated with older methods.

What is Endoscopic Spine Surgery?
Endoscopic spine surgery is essentially "keyhole" surgery for your back. Instead of creating a large opening to see the spine with the naked eye or a microscope, we introduce a thin, high-definition camera—an endoscope—directly to the problem area. Because the surgical instruments and the camera are so small, we do not need to cut or strip your muscles off the bone. Instead, we gently separate the muscle fibers just enough to let our instruments pass through.
This approach minimizes the destabilization of your spine, reduces blood loss, and prevents the heavy scar tissue formation that often follows traditional open surgeries.
Uniportal vs. Biportal: Tailoring the Approach to You
In my practice, I utilize two distinct forms of endoscopic surgery depending on the specific anatomy of your spine and the exact location of the problem.
The first is called Uniportal Endoscopy. As the name suggests, this involves a single tiny incision. The camera and the surgical instruments are housed together in one single tube. I find this technique exceptionally useful for "extraforaminal" issues—meaning pathology that is located on the far outer edges of the spinal canal.
The second technique, which serves as the true workhorse in my operating room, is called Biportal Endoscopy (or UBE). This method involves making two very small incisions, often less than a centimeter each. One portal is dedicated entirely to the camera, while the second portal is used for my surgical instruments. This separation is crucial because it allows me to move my hands and the camera independently. It gives me unparalleled, dynamic 3D-like visualization, allowing me to look around corners and ensure the nerves are completely freed. For a surgeon, it provides the familiar tactile feedback of traditional surgery, but with all the incredible benefits of a minimally invasive approach.
Types of Surgeries We Can Perform
Endoscopic techniques are no longer just for simple disc herniations. The technology has advanced to the point where we can routinely handle complex spinal issues:
- Endoscopic Decompression: If you suffer from sciatica (shooting leg pain) or spinal stenosis (a narrowing of the spinal canal that causes heaviness and pain in the legs when walking), we can use the endoscope to carefully shave away bone spurs, thickened ligaments, or herniated disc fragments. The biportal technique is especially effective for thoroughly decompressing the central spinal canal.
- Endoscopic Lumbar Fusion: For patients who have spinal instability or severe degeneration that requires locking two vertebrae together, we can now perform a Transforaminal Lumbar Interbody Fusion (TLIF) endoscopically. Using the biportal approach, I can clear out the damaged disc, place a supportive cage between the bones, and insert stabilizing screws, all through very small incisions. This achieves the exact same structural result as an open fusion but drastically reduces blood loss and tissue trauma.
What to Expect: Anesthesia and Recovery
There is a growing trend in the media discussing "awake" spine surgery under local anesthesia. While this is technically possible, I generally do not recommend it. Lying perfectly still on your stomach for an hour or more while a surgeon works on your spine requires an immense amount of patience and can cause significant anxiety. My priority is your absolute safety and comfort. Therefore, we typically utilize general anesthesia or deep sedation so you can sleep peacefully and painlessly through the entire procedure.
The true magic of endoscopic surgery is not found in being awake during the operation—it is found in the recovery.
Because we do not traumatize the heavy muscles of your back, the postoperative experience is remarkably different from open surgery. Patients generally mobilize and get out of bed much faster, often on the same day. Hospital stays are drastically shortened, with many patients returning home within 24 hours. Furthermore, the need for heavy, postoperative opioid pain medication is significantly reduced, and patients routinely return to work and their normal daily activities much faster.
Finally, the safety profile regarding surgical infections is astounding. Traditional open spine surgery carries an infection risk of around 1.2%, which climbs even higher for patients who carry extra weight. However, because endoscopic surgery uses tiny incisions and continuous saline irrigation to wash out the surgical site, the infection rate drops to a virtually negligible 0.001%. This makes endoscopic surgery an incredibly safe and highly effective option for patients of all body types, including those with a higher Body Mass Index (BMI).
