As an orthopedic spine surgeon, my ultimate goal is to provide you with a lasting, successful surgical outcome. However, performing a spinal fusion or hardware placement on weakened bone is like trying to anchor a heavy screw into crumbling drywall.

Poor bone health in patients undergoing elective spine procedures is incredibly common — with up to 20% of patients having osteoporosis and 30% to 50% having osteopenia. This underlying bone disease often goes unrecognized and untreated until a devastating surgical complication occurs.

Part One · Diagnosis

Know the four diagnostic concepts.

Four ideas every patient over 50 should understand — diagnosis, the window for prevention, the warning of a first fracture, and the biology of bone loss.

Osteoporosis is a systemic skeletal condition characterized by a dangerous loss of bone mass and the severe deterioration of your bone's internal microarchitecture. Approximately 1 in 2 women and 1 in 4 men over the age of 50 will break a bone due to osteoporosis.

It is often called a "silent disease" because you cannot feel your bones getting thinner. It is almost always completely asymptomatic right up until the moment you suffer your first fracture.

Pathway · DEXA

A dual-energy x-ray absorptiometry (DXA or DEXA) scan measures Bone Mineral Density. Per WHO, a T-score of −2.5 or lower at the lumbar spine, total hip, or femoral neck means osteoporosis.

Pathway · Clinical

You do not need a DEXA scan to be diagnosed. A low-energy fragility fracture — hip or spine from a minor fall — is a clinical diagnosis of osteoporosis regardless of bone density score.

Osteopenia means you have low bone mass. Your bones are weaker than normal, but the bone loss is not yet severe enough to be classified as osteoporosis.

Diagnosis: on a DEXA scan, osteopenia is defined as a T-score between −1.0 and −2.4.

A diagnosis of osteopenia is a critical warning sign — but also an opportunity. You have entered a vital window for prevention. You should aggressively manage your lifestyle to stop further bone loss:

  • Weight-bearing exercise and progressive resistance training
  • Optimize calcium and Vitamin D intake
  • Actively monitor bone density with a new DEXA scan every 1–2 years

A fragility fracture is a broken bone that results from low-energy trauma — such as a simple fall from a standing height or less. Healthy bones do not break from this kind of minor impact.

Most common sites: spine (vertebral compression fractures), hip, wrist, and proximal humerus (shoulder).

The Cascade Effect

After a postmenopausal woman suffers an osteoporosis-related fracture, she is 5× more likely to fracture again within a single year.

Surgical Complications

Poor bone quality increases the risk of intra-op fractures, hardware loosening (screw pullout), and revision surgery.

A Missed Opportunity

Less than half of patients who suffer a fragility fracture receive recommended osteoporosis care or medications. These are sentinel events.

Many people think of the skeleton as a static, dry structure — but your bone is a dynamic, living tissue continuously changing. Your body maintains bone strength through a continuous cycle called bone remodeling, balanced between two cell types:

Osteoclasts

The "demolition crew" that breaks down and resorbs old bone.

Osteoblasts

The "construction crew" that follows behind to form brand-new bone.

Bone loss happens when the demolition crew works faster than the construction crew. Three drivers tip the balance:

  • Hormonal shifts — estrogen keeps osteoclasts in check; menopause accelerates loss
  • Medications — long-term corticosteroids, anti-seizure drugs, PPIs, SSRIs all deplete bone
  • Inflammation & oxidative stress — chronic stress inhibits bone-builders and hyper-activates bone-destroyers

Density vs. quality: bone density (mineral content on DEXA) does not equal bone strength. Bone quality — microarchitecture, collagen framework, and the ability to repair micro-damage — is just as important.

Part Two · Practical Bone Care

What you can actually do.

Testing, prevention, medications, exercise, and nutrition — the building blocks of a real bone-health plan.

Your skeleton is a living, dynamic tissue constantly remodeling itself — a balance between osteoclasts (the demolition crew) and osteoblasts (the construction crew). When the demolition crew outpaces the builders — through aging, menopause, medications, or chronic inflammation — you lose bone mass.

True bone strength is determined by a combination of bone density (how much mineral is in the bone) and bone quality (internal microarchitecture and flexibility). Protecting both is the key to a strong foundation.

A dual-energy x-ray absorptiometry (DEXA or DXA) scan is the gold standard test for measuring bone mineral density (BMD) at the spine, hip, and wrist. Results are given as a T-score, comparing your bone density to a healthy 30-year-old.

However, the DEXA does not tell the whole story. It measures everything in the path of the x-ray — so degenerative spinal arthritis and bone spurs can falsely inflate your score, making your bones look healthier than they actually are.

A fragility fracture is a broken bone resulting from low-energy injury — a fall from standing height or less. If you suffer one, you have clinical osteoporosis regardless of what your DEXA scan says.

An elderly patient who suffers a wrist fracture has a greater than 5× risk of sustaining a devastating hip fracture within the next year. Prevention requires:

  • Fracture Risk Assessment Tool (FRAX) to calculate 10-year risk
  • Appropriate medical management
  • Lifestyle changes
  • Fall reduction strategies

As an orthopedic spine surgeon, I cannot overstate the importance of bone health before entering the operating room. Osteoporosis affects up to 20% of patients undergoing spine surgery and significantly increases the risk of serious complications:

  • Hardware loosening
  • Pedicle screw pullout
  • Implant subsidence
  • Need for revision surgery

Diagnosis & Screening

No. While a T-score of −2.5 or lower on DEXA officially defines osteoporosis, you can also receive a "clinical diagnosis" regardless of your score. A low-energy fragility fracture — hip, pelvis, or spine from a simple fall — means your foundation is compromised. We also use the FRAX score: if your 10-year risk for a major osteoporotic fracture is ≥20%, or hip fracture is ≥3%, you may need treatment even with an osteopenia-range T-score.

A DEXA scan is recommended for all women aged 65+ and all men aged 70+. Younger postmenopausal women and men aged 50–69 should be screened earlier if they have clinical risk factors — previous low-energy fracture, low body weight, smoking, family history of osteoporotic fractures, or long-term glucocorticoid use.

Medications & Treatment

Antiresorptive medications (bisphosphonates, Prolia) act like security guards — they slow osteoclasts that break down bone, maintaining current density. Anabolic medications (Forteo, Tymlos, Evenity) act like a construction crew — they target osteoblasts to stimulate massive new bone formation. For very high fracture risk we use a "Build and Seal" strategy: start with an anabolic to build the foundation, then follow with an antiresorptive to lock in the gains.

The actual risk is extremely low. Atypical femur fractures with bisphosphonates: 3 to 50 per 100,000 patients. Osteonecrosis of the jaw is exceedingly rare in standard osteoporosis treatment. You are statistically more likely to be in a fatal car accident than to suffer either. The real-world danger of leaving osteoporosis untreated drastically outweighs these rare medication risks.

Absolutely not. Stopping or significantly delaying Prolia can cause a rapid "rebound" — bone turnover spikes, rapid bone loss follows, and the risk of multiple severe vertebral compression fractures rises sharply. If you and your doctor decide to stop, you must carefully transition to another medication, like a bisphosphonate, to protect against rebound loss.

Exercise & Lifestyle

Walking is fantastic for cardiovascular health and general mobility, but it represents very low-intensity loading and is largely ineffective for stimulating new bone on its own. To actively challenge your skeleton you need progressive, high-intensity resistance training (lifting weights) and weight-bearing impact exercises (jumping, skipping, jogging). Combining this with balance and flexibility training — Tai Chi, yoga — is highly effective for reducing falls.

Sleep quality and duration are critical pillars of skeletal health. There is a U-shaped relationship: both short sleep (<6 hrs) and long sleep (≥9 hrs) are associated with increased risk of osteoporosis. Chronic sleep deprivation and insomnia overactivate the stress response, elevate cortisol, and trigger systemic inflammation — all of which inhibit bone-forming cells.

Nutrition & Supplements

Women over 50 and men over 70 should aim for 1,200 mg of calcium daily. For adults under those ages: 1,000 mg daily. Get calcium from your diet first (dairy, leafy greens, fortified foods) and only use supplements to fill the gap. For Vitamin D, adults over 50 need 800–1,000 IU daily — higher if blood tests show deficiency.

Calcium carbonate is the most common and least expensive, but it requires an acidic stomach environment to absorb — so it must be taken with meals. If you take PPIs or H2 blockers for acid reflux, your stomach acid is reduced and absorption suffers. In these cases, calcium citrate is the first-line supplement because it absorbs easily on an empty stomach.

As an orthopedic surgeon, I need strong bone to anchor your implants. Poor bone quality significantly increases risk of intra-operative fractures, hardware loosening, pedicle screw pullout, and implant subsidence. Optimizing bone health before elective orthopaedic surgery — often by initiating anabolic bone-building medications — can dramatically improve outcomes, speed up fracture healing, and prevent revision surgeries.
